The Linkage information hub ensures people in Wellington city, Porirua and Kapiti Coast are supported to find services and community based groups and activities that meet their health and wellbeing needs.
We work with frontline health and social service workers, needs assessment and service coordination (NASC) and service providers as well as people who access services and their family/whānau.
Our information hub coordinator helps connect people with information about services and supports available in their communities. The coordinator also works alongside the district councils and Capital and Coast District Health Board to maintain an online directory and increase people’s access to information.
Our community capacity liaison works with local services and groups in the community to understand and help break down the barriers people may face when accessing services and community organisations.
